Registration Procedures

 

1. USA Hockey Annual Membership & Registration
   • Register online with USA Hockey for 2010-11 Season - USA Hockey On-line Registration
     • Select "Ice Players & Coaches"
     • Use barcoded receipt number in NYHL online registration (step #2)
2. NYHL Registration
   • Register Online with NYHL - NYHL On-line Registration
   • Print & Complete NYHL Forms below
     • Player Code of Conduct
     • Consent-to-Treat / Medical History 
     • USA Hockey Waiver
   • Deliver Completed Forms to NYHL registration table during NYHL Pre-season (House League Evaluations) or team manager if on a Southern Flyers travel team.  
   • If beyond the pre-season, deliver to the NYHL via either of the following:
